Tips on Completing Prescriptions
To avoid delays, always include the following information on all scripts:
Prescriber Info
First Name, Last Name, Address, Phone & Fax, State and DEA License, Office contact name (not required, but preferred).
Patient Info
First Name, Last Name, Address, Date of Birth & Cell Phone Number/Email (this is imperative for patients to receive initial notification of a prescription), Drug Allergies.
Complete Prescription Information
Medication Name, Dosage Form, Strength, Directions, Quantity, and Refills.
Route of Administration
Oral, IV, SQ, IM.
Shipping Method
Ground (3-5 Business days), 2nd day, Standard overnight (By end of following business day), Priority Overnight (following business day Morning delivery), US Mail (PO BOX only and no estimated time of arrival), signature option (required or no signature
(waives liability).

Bill clinic or Bill patient
Prescriber Signature
Please provide an authentic prescriber signature for all controlled substances. Stamps and electronic signatures faxed in are not considered valid by the DEA.
Supplies are Prescriptions Too!
It is required by the Florida Board of Pharmacy and therefore by APS Pharmacy that all supplies, including but not limited to:
- Needles
- Syringes
- Mixing Kits
- Sharps Containers
Must be written on the prescription in order to be filled. To avoid delays, please include the size and quantity of the desired supplies.

APS is a sterile pharmacy, yet we offer a wide variety of sterile and non-sterile medication forms.
